New Patients
If you were born outside of New Zealand, please provide a passport, proof of residency, or a working visa endorsement. Australian citizens or permanent residents must also provide evidence that they intend to stay in New Zealand for at least two consecutive years.
Once all originals have been sighted and verified by our reception staff, an appointment can be booked with your GP.
To enrol with us, please discuss the paperwork required with Reception.
Guardians / Parents
Guardians/parents of children under the age of 16, have automatic access to the child’s medical records. However, once the child turns 16 years of age, this access will be removed as per privacy health standards (NZ).
A consent form can be signed to authorise someone else to have access to the patient portal.
